Release PR. It promotes the v0.12.9..main payload to latest as v0.12.10: #1669 "fix: workspace rules state lives outside the project", #1670 "fix(rules): a project linked by path can gain its git remote, and learn status says why rules are off", #1687 "fix: tool commands no longer lose a big answer on a pipe" and #1688 "fix(session): retry a failed session title on later turns and report why it failed" (closes#1689).

What does this PR do?
Adds the 0.12.10 changelog entry, the release adversarial tests (packages/opencode/test/skill/release-v0.12.10-adversarial.test.ts, 18 cases) and the fixes from the release review, on top of the four merged fixes above.
Review fixes in this PR (from five persona reviews of the real diffs, all SHIP WITH NOTES, no P0):
tool commands: writeAll retried EAGAIN forever, so a reader that stopped draining the pipe kept the process spinning. After 60 s without progress it now fails with a message on stderr and exit 1. EPIPE still exits 0. (packages/opencode/src/cli/cmd/tool.ts)
Session titles: a title_generation_failed report for a small_model the provider does not list named the chat model it fell back from. It now names the model that was asked for. The fire-and-forget title call has a .catch. A new test keeps a name the user gives the session while the title request runs. (packages/opencode/src/session/prompt.ts, test/session/title-retry.test.ts)
Docs: the models page says where a failed small_model shows up.
Deferred, with issues:#1691 "Workspace rules state is not carried over from the 0.12.9 in-project rules.json, and nothing tells the user" and #1692 "Session title attempt counter reads the compacted history, so compaction resets the five-attempt bound and the title context". The changelog states the upgrade effect of #1669 in full.
How did you verify your code works?
Preflight (bun script/release-preflight.ts --version 0.12.10 --stage pre): 9 PASS, 0 WARN, 0 FAIL on a clean tree at origin/mainb06bd16bde.
Five persona reviews (CTO, Product Manager, End User, Tech Lead, on-call support engineer as the Chaos Gremlin) read every diff: 5 SHIP WITH NOTES, 0 HOLD, 0 P0. Actionable items are the fixes above; the rest is wording in the changelog or filed as the two issues.
bun run typecheck clean. New adversarial file 18 pass. test/session/title-retry.test.ts 5 pass. test/altimate/tool-cli and test/telemetry/telemetry.test.ts 237 pass.
Full suite and end-to-end checks against the free tier and freemium are running; results will be added here before the merge is requested.
Already verified for fix(session): retry a failed session title on later turns and report why it failed #1688 on its own PR: end to end on the free tier with a built 0.12.10 binary (fresh registration titled a session; an unlisted small_model logged ProviderModelNotFoundError and left the placeholder; a later message retried and titled the session), and the title_generation_failed events reached App Insights.
Cost side effects
None new in this PR. #1688's: at most five title attempts per session whose title request fails, each with up to two SDK retries, each the size of today's single title request; none for a session titled on the first message.

Promotes v0.12.10 to latest: four bug fixes from main, plus the changelog entry, adversarial release tests, and fixes from the release review.
tool writes that stall for 60 seconds now exit 1 with a stderr message instead of spinning forever; a reader that closes early still exits 0. The stall is measured on a monotonic clock, and zero-byte writes count as no progress.
Title failure telemetry now names the model that was asked for, not the chat model it fell back from, and the fire-and-forget title call catches errors.
The models docs state where a failed small_model shows up.
Written for commit 8b42660. Summary will update on new commits.

New Features
Workspace rules are available after sync and stored privately per user; the in-project approved copy is display-only. Existing workspace rules are not migrated.
learn status now explains workspace-rule states, and relinking a path-linked project can detect its Git remote.
Bug Fixes
Session titles retry after each of the first five user messages. Title-generation failures are logged and reported.
Tool commands deliver complete output; a stalled reader times out after 60 seconds, while an early-closing reader exits successfully.
…ilures, and catch the title call
Release review findings for v0.12.10:
- `tool` commands: `writeAll` retried EAGAIN forever, so a reader that stopped
draining the pipe kept the process spinning. After 60 s without progress it
now fails with a message on stderr and exit 1. EPIPE still exits 0.
- Session titles: a `title_generation_failed` report for a `small_model` the
provider does not list named the chat model it fell back from. It now names
the model that was asked for (the title agent's own, or `small_model`).
- The fire-and-forget `ensureTitle` call has a `.catch`, so a throw before the
request can never be an unhandled rejection.
- Tests: a name the user gives the session while the title request runs is
kept; the unlisted-small-model test asserts the requested model id.
- Docs: the models page says where a failed `small_model` shows up.

The release adds stalled-write handling for tool output and updates session-title request and failure reporting. It also adds release notes and adversarial tests covering workspace-rule state, project linking, status output, and these fixes.
writeAll tracks progress, retries writes that make no progress, and throws after 60 seconds without progress. Tests check pipe errors, stalled writes, and stdout usage.
Session-title retries and failure reporting packages/opencode/src/session/prompt.ts, packages/opencode/test/session/title-retry.test.ts, packages/opencode/test/skill/release-v0.12.10-adversarial.test.ts, docs/docs/configure/models.md
Title requests run without blocking the prompt loop. Failure reporting prefers the requested model IDs when available. Tests cover failure reporting and a user-set title during an in-flight request. The documentation describes the failure log and telemetry event.
Release notes and workspace-rule checks CHANGELOG.md, packages/opencode/test/skill/release-v0.12.10-adversarial.test.ts
The changelog describes workspace-rule state, project linking, status output, and the release fixes. Tests check workspace-state paths, link conditions, and learn status output.
The output fix is mergeable with a test follow-up: automated tests do not yet exercise its 60-second stall deadline or zero-byte-write behavior.

+- **A session keeps `New session - <time>` only while its title request keeps failing.** The title is retried on each of the first five user messages, always from the first message; a session titled on the first message makes no extra request, and one that already has more than five messages (for example resumed from an older version) is left as it is. Each failure writes `failed to generate title` to the log with the provider's error (`errorType`, `message`, `status`, `attempt`) and sends a `title_generation_failed` telemetry event (provider and model that were asked for, attempt, error class and type, HTTP status; no message text). The worst case for a session whose small model never answers is five attempts of up to three HTTP requests each. A `small_model` the provider cannot serve, or does not list, is a likely cause; the models page now describes it.

End to end against freemium (api.myaltimate.com, test tenant, an isolated HOME and XDG directories, fresh project folders, the 0.12.10 binary built from 02e89fb273 with the production Base gateway URL embedded):
Workspace rules: off — this project is linked by its path, and workspace rules need a git remote on the link (add a remote if the project has none, then run altimate-code link).
git remote add origin …, then link --workspace <name> --yes
Link kept, remote stored: workspace status now says Matched by git remote. learn sync then reports Synced: 0 team rules, 0 pending review. and learn status shows the workspace name and last sync time.
One file at <state>/altimate-code/workspace-rules/<64-hex>.json, mode 0600, in a 0700 directory. The project holds only .altimate-code/learn/workspace-rules/approved.json and its .gitignore; no rules.json.
Verified on its own PR: fresh Base registration titled a session; an unlisted small_model logged ProviderModelNotFoundError and left the placeholder; a later message retried and titled the session; title_generation_failed reached App Insights.
Not covered here: the TUI link picker's "adding the remote" toast (the headless and --workspace paths were exercised), Windows, and the 60 s stall deadline added in this PR, which is being checked separately and will be added below.
Left behind on the test tenant: workspace 55, e2e-v01210-11070023, with no rules.
Stall deadline check (the writeAll fix in this PR):tool list --json piped to a reader that did not read for 75 s. The command exited 1 after 60 s with stdout stopped accepting output for 60 s; 2539 bytes of the answer were not written on stderr (behind the CLI's Error: Unexpected error prefix), and the reader then received the 65,536 bytes the pipe buffer held. Before this change the process would have spun at 2 ms intervals until the reader returned.
Local full suite, in progress: the only failures so far are test/server/httpapi-mcp.test.ts "serves status endpoint" and five test/mcp/headers.test.ts cases, which fail identically on the pre-release tree on this machine (powershell not found in their output), so they are environmental here and not from the release. CI is the evidence for those.

Wait for the title check before asserting.
If title processing takes longer than 500 ms, both assertions can pass before the placeholder re-check. Observe the successful response reaching that check and wait for completion with a timeout.
As per coding guidelines: “Wait on a published readiness signal, not wall-clock time.”

Use a monotonic clock for the stall deadline.
If the system clock moves backward while stdout is stalled, writeAll can wait longer than 60 seconds. Use a monotonic clock for all three timestamps, and update the source assertion in the release test.

Make writeAll non-blocking before writing to stdout.
When the tool CLI runs under Bun 1.3.14 and a pipe reader stops draining, Bun forces POSIX process.stdout into synchronous mode. writeSync can then block inside the kernel, so the 60-second check is never reached.

Review comment at @packages/opencode/src/cli/cmd/tool.ts at line 317:
Update the stdout-writing path in writeAll so process.stdout remains
non-blocking before any writes occur, including under Bun 1.3.14 when a pipe
reader stops draining. Use a supported mechanism that prevents synchronous
kernel writes, and preserve the existing WRITE_STALL_MS timeout behavior.

Local full suite on 02e89fb273 (OPENCODE_SKIP_SUBPROCESS=1 bun test --timeout 90000, 17 minutes): 20,708 pass, 954 skip, 78 todo, 6 fail across 954 files, no unhandled errors between tests. The 6 failures are the MCP ones noted above (test/server/httpapi-mcp.test.ts status endpoint and five test/mcp/headers.test.ts cases), which fail the same way on the pre-release tree on this machine. The subprocess pass (test/cli/acp, smokes, serve, run-process, mcp-add, help-snapshots) was left to CI.
…ro-byte writes as no progress
Bot review findings on the release PR:
- `writeAll` timed its 60 s stall bound with `Date.now()`; a clock set back
could extend it. It uses `performance.now()` now, and a zero-byte write no
longer counts as progress, so a sink that accepts nothing also hits the bound.
- The release adversarial test sets `OPENCODE_TEST_STATE_HOME` before any
module reads the state directory, and skips the symlink case on Windows.
- Changelog: the `learn status` reasons apply before the first sync.

Bot findings on 02e89fb273, where they landed (one more commit, CI rerunning):
Fixed
writeAll stall bound now uses performance.now(), and a zero-byte write counts as no progress rather than success.
The adversarial test redirects OPENCODE_TEST_STATE_HOME before any module reads the state directory, and the symlink case is skipped on Windows.
Changelog: the learn status reasons are stated as applying before the first sync, which is what the code does.
Not changed, by design
A full blocking stdout pipe blocks inside writeSync instead of raising EAGAIN, so the 60 s bound does not apply there. That is the ordinary behaviour of any program writing to a blocking pipe, and the process resumes as soon as the reader does; the bound exists for the non-blocking case, where the process used to spin. Verified on macOS, where the stall produced EAGAIN and the command exited 1 after 60 s.
The rename-race test waits a fixed 500 ms after releasing the held title answer. The production path exposes no handle for the fire-and-forget request, and a longer wait only slows the suite; the assertion would fail during that window if the guard were removed.
226-228: Trivial · Maintainability & Code Quality · Heavy lift
Test stalled and zero-byte writes through writeAll.
The source-regex assertions do not check control flow or the timeout result. The existing CLI tests cover delayed readers and early departure, but not the 60-second deadline or zero-byte writes.
Add behavioral tests with a controlled writer and clock. Check that repeated EAGAIN and zero-byte writes reach the deadline, and that progress resets it.

Adds the
0.12.10changelog entry, the release adversarial tests (packages/opencode/test/skill/release-v0.12.10-adversarial.test.ts, 18 cases) and the fixes from the release review, on top of the four merged fixes above.Review fixes in this PR (from five persona reviews of the real diffs, all SHIP WITH NOTES, no P0):
toolcommands:writeAllretried EAGAIN forever, so a reader that stopped draining the pipe kept the process spinning. After 60 s without progress it now fails with a message on stderr and exit 1. EPIPE still exits 0. (packages/opencode/src/cli/cmd/tool.ts)title_generation_failedreport for asmall_modelthe provider does not list named the chat model it fell back from. It now names the model that was asked for. The fire-and-forget title call has a.catch. A new test keeps a name the user gives the session while the title request runs. The changelog states the upgrade effect of #1669 in full.
