Problem

A scheduled run has no pull request to turn red, and GitHub emails its failure to one person: whoever last edited the cron line. A push run's failure goes to whoever pushed. Four workflows could therefore fail without the team noticing:

  • fuzz.yml: the nightly fuzz campaign uploads the crash input as an artifact and does nothing else.
  • test-eql.yml: the full PG 14-17 matrix, on push to main and nightly.
  • macro-expand-eql.yml: the nightly check that the cargo expand snapshots haven't drifted.
  • bench-eql.yml: the benchmarks, on push to main and nightly.

Only musl-build-image.yml already reports its failures, by opening an issue.

Change

  • New reusable workflow _report-unattended-failure.yml. It opens an issue titled <workflow> failed on <branch>, labelled needs-triage. If that issue is still open, later failures are added to it as comments, so each workflow has one thread rather than a new issue every night. The issue body and each comment link the run, name the event and commit, and include the calling workflow's instructions for what to do.
  • Each of the four workflows ends with a report job. The job needs every other job in its workflow. Its condition is failure() && github.event_name != 'pull_request' && github.event_name != 'workflow_dispatch'.
    • The condition excludes events rather than allowing only schedule. Stack's own tests reject conditions that list allowed events, because a trigger added later would be skipped silently.
    • As a result, push-to-main failures of test-eql and bench-eql are reported too. Their run is the only check of the merged result, and it was going just as unseen.
    • Manual runs are excluded because the person who started one is watching it, and may have run it on another branch.
  • New scripts/__tests__/unattended-failure-report.test.mjs. It scans .github/workflows and requires:
    • every scheduled workflow to call the reporter, or be listed with a reason (codeql, osv-scanner, musl-build-image);
    • each report job's needs to list every other job in its workflow. Otherwise a job added later could fail without being reported.
    • the job condition and permissions to match the spelling above exactly.
  • lib/expressions.mjs now understands failure(). The result must be supplied by the caller; if it isn't, evaluation throws, as for any other expression it can't evaluate. workflow-dispatch-job-conditions.test.mjs sets failure() to true, the same way it sets job outputs to their passing values. The four report jobs are added to DISPATCH_SKIPPED_JOBS, with the reason.
  • docs/fuzzing.md now says where fuzz failures are reported.

Testing

  • pnpm test:scripts: everything I changed or touched passes. Two suites, bench-index-expressions and check-auth-npm-changeset, fail to load in my local worktree because of missing local dependencies. They fail the same way on an unmodified main checkout.
  • actionlint with shellcheck reports nothing on the five workflow files.
  • I ran the issue lookup query (gh issue list --search ... --jq 'map(select(.title == env.TITLE))') against this repo's live issues. It finds an exact title match and returns nothing when there's no match.
  • The report job itself can't run until a scheduled run or push run on main fails after this merges.

Summary by CodeRabbit

  • New Features
    • Qualifying benchmark, fuzzing, macro-expansion, and test workflow failures can now create an issue with troubleshooting guidance and a link to the run. If an open issue already exists for the same workflow and branch, the failure is added as a comment instead.
  • Documentation
    • Fuzzing guidance now explains how scheduled-run failures are reported.

📝 Walkthrough

Four GitHub Actions workflows now invoke a reusable reporter for qualifying failures. The reporter comments on an open issue with an exact workflow-and-branch title or creates a labeled issue. Tests check caller configuration, scheduled-workflow coverage, and failure-condition evaluation.

P2 Badge Serialize issue lookup and creation

When two push runs of the same workflow/ref fail concurrently (particularly the hour-long bench-eql runs), both reporters can finish the lookup before either creates the issue, take this else branch, and create duplicate issues with the same title. This violates the stated one-open-issue-per-workflow-and-branch behavior; serialize reporters using a concurrency key derived from the workflow/ref and account for search-index lag when performing the lookup.

Fixed in e9fef68. The report job has a concurrency group per workflow and branch (report-unattended-failure-<workflow>-<ref_name>) with cancel-in-progress: false, so a second reporter waits and then finds the first one's issue. The lookup now lists issues directly rather than through the lagging search index. One limit remains: GitHub keeps only one pending run per group, so if three reports overlap, the middle one is dropped. The comment in the workflow says so.

Blocking: test-eql.yml and fuzz.yml fail at startup on this head

On fee9dc52, two workflows did not start:

  • Test EQL, run 37897081288: startup_failure
  • Fuzz (crates), run 37897081235: startup_failure

gh pr checks 1146 does not show these runs, because a startup failure creates no check run. Thus the PR looks green, but it is not. On main, this breaks Test EQL on every pull request, push and nightly run, and ci-required never reports. The two workflows that failed are the two that this PR changed and that trigger on pull_request. bench-eql.yml and macro-expand-eql.yml have no pull_request trigger, so this PR did not exercise them. Expect the same failure there.

The API does not give the startup error text. The most probable cause is the permissions handoff to the called workflow (see the inline comment). Open run 37897081288 in the browser to read the exact message before you change the code.

After the fix, push again and confirm that Test EQL and Fuzz (crates) start on the PR. actionlint does not detect this class of error, and the new test does not either.

Points already raised

I agree with the open points from CodeRabbit (--limit on the issue lookup) and Codex (concurrent reporters can open two issues). I do not repeat them here.

Suggested follow-up PR

  • test-eql.yml has merge_group: {} (inert today). The report condition does not exclude it. If the repo turns on a merge queue, github.ref_name is gh-readonly-queue/main/pr-<n>-<sha>. Each queue failure then opens a new issue with a unique title, and the PR author already sees that failure. Add github.event_name != 'merge_group' to the condition, or write down why it is acceptable. The test pins the condition text at scripts/__tests__/unattended-failure-report.test.mjs:35, so update it in the same change.
  • Add a guard to unattended-failure-report.test.mjs that checks the called workflow requests no permission that the caller job does not grant. That is the property that failed here, and nothing in the repo checks it.

No changeset is necessary. This is CI-only.

Probable cause of the startup failures. The caller job grants only issues: write (for example test-eql.yml:1056-1057), so every other scope, contents included, is none for the called workflow. This file requests contents: read at the workflow level. A called workflow can only reduce the permissions it receives, and GitHub rejects the run at startup when it asks for more.

Do one of these:

  • Remove the workflow-level block, or set it to permissions: {}. The job needs only issues: write, and gh issue list / gh issue create with GH_REPO do not need contents.
  • Or grant contents: read in each of the four caller jobs. Then change scripts/__tests__/unattended-failure-report.test.mjs:108, which pins { issues: 'write' }.

Confirm the exact message on run 37897081288 first. If the message names another cause, fix that instead.

Confirmed and fixed in f8e012b. The run page for 37897081288 shows: "Error calling workflow '.../_report-unattended-failure.yml@...'. The workflow is requesting 'contents: read', but is only allowed 'contents: none'." Fuzz run 37897081235 shows the same text. The reporter now has permissions: {} at the workflow level. Its job keeps issues: write and needs no contents. unattended-failure-report.test.mjs now checks that each caller's report job grants every scope that the reporter asks for, at the workflow level and in each job. It fails on the old file.

Addressed the review from @freshtonic. I rebased the branch onto main.

  • Blocking, startup failure: fixed in f8e012b. The run page gave the cause: "The workflow is requesting 'contents: read', but is only allowed 'contents: none'". The reporter now declares permissions: {} at the workflow level. On the new head (e9fef68), Test EQL (run 39126324613) and Fuzz (crates) (run 39126324625) start and create jobs. No run for this commit has startup_failure. bench-eql.yml and macro-expand-eql.yml call the same file with the same grant, so the same fix covers them. They have no pull_request trigger, so this PR cannot start them.
  • Permission guard (follow-up): this is in f8e012b too. unattended-failure-report.test.mjs fails when the reporter asks for a scope, at the workflow level or in a job, that any caller's report job does not grant. With the old contents: read block it fails for all four callers.
  • merge_group (follow-up): e9fef68 adds github.event_name != 'merge_group' to all four conditions. It updates the pinned condition and the reporter's header comment.
  • CodeRabbit --limit and Codex duplicate issues: e9fef68 lists open issues directly instead of using --search, and adds a concurrency group per workflow and branch. See the inline replies.

Local checks: the full scripts/ vitest suite gave 78 files and 1368 tests passed. actionlint 1.7.7 is clean on the five changed workflows. Biome is clean.

Comment thread .github/workflows/macro-expand-eql.yml Outdated
A scheduled run, or a push to main, has no pull request to turn red, and
GitHub mails its failure to one person. The nightly fuzz campaign, the
EQL matrix, the macro-expand drift check and the EQL benchmarks could
all fail without anyone noticing. Each now ends with a job that opens an
issue, or comments on the open one, through a shared reusable workflow.

The report job runs on every event except pull_request and
workflow_dispatch rather than on schedule only, so it also covers
post-merge failures and any trigger added later, and satisfies the
existing rule against job conditions that enumerate events.

A new test requires every scheduled workflow to call the reporter or
give a reason, and every report job to need all other jobs. The
expression evaluator learns failure(), which callers must set
explicitly.
Test EQL and Fuzz (crates) stopped starting on this branch. Both runs
ended in startup_failure with "Error calling workflow
'.../_report-unattended-failure.yml'. The workflow is requesting
'contents: read', but is only allowed 'contents: none'." A called workflow
gets at most the calling job's grant. The report jobs grant only
`issues: write`, and GitHub checks the called file's workflow-level block
against that grant even when its job sets its own. bench-eql.yml and
macro-expand-eql.yml call the same file and would fail the same way on
their first push or scheduled run.

The reporter now declares `permissions: {}` at the workflow level. Its job
reads and writes issues through GH_REPO and needs no checkout, so
`issues: write` is all it asks for.

A startup failure creates no check run, so the PR looked green, and
actionlint does not model the handoff. unattended-failure-report.test.mjs
now holds every caller's grant over every scope the reporter asks for, at
the workflow level and in each job, and fails on the shape that broke.
Three gaps in the reporter's "one open issue per workflow and branch":

- The lookup used `gh issue list --search`, which returns 30 ranked
results by default and reads a search index that lags. The right issue
could fall outside the window, and an issue opened moments earlier
could be missed. It now lists every open issue directly and matches the
  exact title.
- Two failed runs of one workflow on one branch could both look, both
find nothing and both open an issue. The report job now has a
concurrency group per workflow and branch, without cancelling, so the
  second finds the first's issue.
- test-eql.yml has a merge_group trigger. A merge queue run's ref is
unique per attempt, so each failure would open a new issue, and the
author already sees the failure on the merge. The report condition now
also excludes merge_group in all four callers.
